The Pedemontana Smart Care integrated system for remote assistance, monitoring and local emergency response

Strategic Framework for Strengthening Home-Based Health and Social Care Services Across the Five Municipalities of the Pedemontana Parmense Union. The project aims to improve the quality, timeliness, and continuity of care provided to vulnerable individuals through the implementation of an integrated system of telecare, remote monitoring (including outside the home environment), and teleassistance services. The initiative also includes the introduction of the home care operator role to strengthen care coordination and intervention capacity across the community. The project is supported by Fondazione Cariparma under the “Welfare Connesso 2026” funding programme.
